Bloccanti del recettore dell’angiotensina II (ARB,  Angiotensin Receptor Blockers) detti anche sartani. Losartan (es. Losaprex®, Lortaan®), Valsartan (es. Tareg®, Combisartan®), Candesartan (es. Blopress®, Ratacand®), Irbesartan (es. Aprovel®, Karvea®), Telmisartan (es. Micardis®, Pritor®), Olmesartan (es. Olpress®) ed Eprosartan (es. Tiartan®).

Si trovano anche in associazione con altri farmaci, come i diuretici e i calcio-antagonisti. Combisartan, Cotareg; Coaprovel, Coabesart; Olprezide, Olmegan, Plaunazide; Losazid, Losartan HCT, Lorcombi; Blopresid, Ratacand plus; Micardis Plus; Tiartan.

Utilizzati nella terapia dell’ipertensione arteriosa, della nefropatia diabetica, steatosi epatica e nell’insufficienza cardiaca specialmente nei soggetti intolleranti agli ACE inibitori. Agiscono sul sistema renina-angiotensina bloccando il recettore AT1 su cui normalmente l’angiotensina II agisce. Agiscono anche sui  recettori AT2, sebbene in misura molto minore. I recettori AT1 quando vengono attivati, inducono vasocostrizione, rilascio di aldosterone e riassorbimento di sodio e acqua, tutti fenomeni ipertensivanti. La stimolazione dei recettori AT1 inducono inoltre liberazione di radicali liberi dell’ossigeno e proliferazione endoteliale con effetto aterosclerotico (13-16).  Il legame di Angiotensina II all’AT1R induce l’accoppiamento a Gαql’attivazione della fosfolipasi C e la mobilizzazione del calcio con conseguente vasocostrizione e aumento della pressione arteriosa.

I recettori AT1oltre che dall’angiotensina II  sono stimolati da deficit estrogenico, ipercolesterolemia e iperinsulinemia.

Effetti della stimolazione dei recettori AT 1 

    1. Vasocostrizione: Provocano il restringimento dei vasi sanguigni, aumentando le resistenze periferiche e quindi riducendo la gittata cardiaca e inducendo ipertrofia dei miociti soprattutto a carico del ventricolo sinistro.
    2. Aumento della pressione arteriosa:  tramite vasocostrizione, ipersecrezione ipofisaria di ADH, ritenzione di sodio e acqua, iperescrezione di  potassio. ipersecrezione di aldosterone.
    3. Ritenzione di Sodio e Acqua:  gli ARB Stimolano la produzione di aldosterone e il riassorbimento di sodio e acqua nei reni, aumentando il volume sanguigno.
    4. Effetti Cardiaci: Aumentano la contrattilità del miocardio (effetto inotropo positivo), tachicardia e ipertrofia cardiaca e infine insufficienza cardiaca congestizia.

I recettori AT2 invece sono dotati di effetti spesso opposti (vasodilatazione, effetto antinfiammatorio, vasoprotezione, effetti neuroprotettivi e  importanti nello sviluppo tissutale, specialmente fetale. Inoltre promuovono angiogenesi tramite la stimolazione della secrezione di bradichinina e NO con effetti benefici su Alzheimer e vascolapatie cerebrali.

Recentemente sono stati identificati, ma non ancora ben caratterizzati, altri due tipi di recettore AT: il tipo 3 e il tipo 4.

SARTANI – meccanismo e sedi di azione
  • Vasodilatazione: Impediscono all’angiotensina II di legarsi al recettore AT1, riducendo la vasocostrizione e le resistenze periferiche.
  • Riduzione della Pressione Arteriosa: Agiscono direttamente sulle pareti arteriosa riducendo la vasocostrizione. Non inducono accumulo di bradichinina e quindi particolarmente indicati per sostituire gli ACE inibitori nei pazienti ipertesi con tosse e angioedema, 
  • Azione Renale: aumentano la perfusione renale con un maggior afflusso derivato dalla dilatazione delle arterie glomerulari afferenti e  una minore pressione intraglomerulare per effetto della vasodilatazione delle arteriole efferenti. Riducono la proteinuria e prevengono complicanze in pazienti con nefropatia diabetica o insufficienza renale cronica. Indipendentemente dalla presenza o meno di ipertensione, gli ARB svolgono un ruolo importante nel trattamento della malattia renale cronica (MRC), in particolare in presenza di albuminuria, per rallentare la progressione della malattia renale e ridurre il rischio cardiovascolare.● Per le persone con diabete di tipo 1 o di tipo 2, offrire un ARB autorizzato per la nefropatia diabetica (o un ACE-I), titolato alla dose massima autorizzata tollerata, se il rapporto albumina:creatinina urinaria (uACR) è pari o superiore a 3 mg/mmol.● Per le persone senza diabete, offrire un ARB (o ACE-I) titolato alla dose massima tollerata autorizzata se uACR è ≥70 mg/mmol. In presenza di ipertensione, offrire se uACR è >30 mg/mmol.

    ● Le linee guida ADA raccomandano un ARB (o ACE-I) per le persone con diabete che presentano albuminuria per prevenire la progressione della malattia renale. 

  • Protezione Cardiovascolare: Impiegati nel trattamento dello scompenso cardiaco, insufficienza cardiaca congestizia, nel post-infarto e nell’ipertrofia ventricolare sinistra  mediante vasodilatazione che riduce il postcarico, consentendo un aumento della gittata cardiaca in caso di insufficienza cardiaca congestizia. Il valsartan in combinazione con sacubitril (Entresto® cpr 24+26 mg, 49+51 mg, 97+103 mg), inibitore della neprilisina,  è raccomandato come opzione per l’insufficienza cardiaca sintomatica (sintomi di classe NYHA II-IV) con frazione di eiezione ridotta (35% o meno) in persone precedentemente sottoposte a terapia con ARB o ACE-I.
  • Trattamento antineoplastico complementare –  Dato che l’attività AT1R dipendente da AngII regola direttamente l’angiogenesi, il rimodellamento della vascolarizzazione, le risposte pro-infiammatorie, la programmazione delle cellule staminali e l’ematopoiesi, nonché l’equilibrio elettrolitico, la modulazione di questi processi con farmaci farmacologicamente ben caratterizzati potrebbe rappresentare una preziosa opzione di trattamento complementare per i pazienti oncologici (32-37).

Usi off-label dei sartani

La terapia profilattica con ACE inibitori o ARB in soggetti con tumore al seno sottoposti a chemioterapia riduce significativamente il declino della frazione di eiezione ventricolare sinistra (FEVS). Tuttavia, questo approccio non riduce significativamente l’incidenza complessiva di cardiotossicità e sono necessarie ulteriori ricerche per comprendere meglio questo effetto (38).

Gli ARB, in particolare il losartan, migliorano significativamente la funzionalità epatica nei pazienti con steatosi epatica non alcolica, nota anche come malattia epatica steatosica associata a disfunzione metabolica. Questo miglioramento si ottiene riducendo i livelli di transaminasi glutammico-ossalacetica. Tuttavia, gli ARB hanno un effetto limitato sui profili lipidici e sono necessarie ulteriori ricerche per comprenderne appieno il ruolo nella gestione della steatosi epatica non alcolica (39). 

Gli ARB e i CCB sono utilizzati anche per trattare il fenomeno di Raynaud grazie ai loro effetti vasodilatatori. Nei casi più gravi, tuttavia, il trattamento può richiedere inibitori della fosfodiesterasi di tipo 5 (PDE-5) o altre terapie avanzate (40).

Secondo le linee guida AHA/ACC, gli ARB dovrebbero essere utilizzati nei pazienti con infarto miocardico recente e frazione di eiezione ventricolare sinistra (FEVS) pari o inferiore al 40%, intolleranti agli ACE-inibitori. Questi agenti aiutano a prevenire l’insufficienza cardiaca sintomatica e a ridurre il rischio di mortalità.

Effetti collaterali – Avendo un meccanismo di azione simile a quello degli ACE–inibitori, anche gli effetti collaterali sono simili. I più diffusi sono:

  • Cefalea
  • Iperkalemia  specialmente se utilizzati in combinazione con diuretici risparmiatori di potassio o integratori di K
  • Vertigini
  • Ipotensione ortostatica
  • Astenia
  • Nausea e vomito
  • I sartani sono farmaci teratogeni, quindi controindicati in gravidanza e in pazienti che ne desiderano una a breve.

Misure precauzionali – Durante la terapia con sartani  è consigliabile ridurre alimenti ricchi di potassio come banane, agrumi, pomodori, patate. Dal punto di vista farmacologico, il contemporaneo utilizzo dei FANS (ibuprofene, naprossene) può ridurre l’effetto antipertensivo dei sartani e aumentare il rischio di danno renale, soprattutto negli anziani o nei pazienti con funzione renale compromessa.
